Diabetic Screenings

Our team will support you in building a healthier patient population.  Our retinal screenings  will detect diabetic retinopathy as early as possible.  This early detection screening will aid in saving vision and be able to guide primary care in the best treatment plan for the patient.  It will also keep you on the path to meeting your HEDIS/ STAR measures and maximizing your reimbursements . Hypertensive Rentinopathy Screenings

Much like diabetes, hypertension can also cause serious issues within the retina and body. High blood pressure can cause the blood vessel walls to thicken, resulting in restricted blood flow.  This damage can cause occlusions of the blood vessels and retinal edema. 

HEDIS

By implementing a convenient retinal screening as part of the routine visit, primary care providers can take control of their diabetes HEDIS measures and obtain payer-based financial incentives. 

As seen below the retinal eye exam is the only component of the assessment that can't be completed at the time of the primary care visit. Jolly Retinal Imaging  solves this problem by conveniently bringing the exam to the primary care providers office, thus completing  the retinal exam requirement. This can save a tremendous amount of time that is currently used for calling patients to remind them to get their diabetic eye exam, nursing and administrative staff trying to get the proper reports back from the eye doctors office and the time it takes to manually match the chart to the patient and enter it into the EMR.  


Diabetic HEDIS Requirements


  • Assesses adults 18–75 years of age with diabetes (type 1 and type 2) who had each of the following:  Hemoglobin A1c (HbA1c) testing. 
  • HbA1c poor control (>9.0%). 
  • HbA1c control (<8.0%). 
  • Eye exam (retinal) performed. 
  • Medical attention for nephropathy.* 
  • BP control (<140/90 mm Hg).

Primary care  staff take a simple retinal photo of each eye. This should take less than 1 minute. Photos are automatically sent to our reading center.

State licensed and credentialed doctors remotely read the images. They then create a report to send back to the PCP's office.
